What is Raktamoksham?
Raktamoksham (Raktamokshana) is the Ayurvedic practice of therapeutic bloodletting — a controlled removal of small quantities of impure blood from the body. It is one of the Panchakarma procedures, specifically indicated for disorders caused by vitiated Rakta (blood tissue) and Pitta dosha. How It Works
When Pitta dosha vitiates the blood tissue (Rakta Dhatu), toxins accumulate in the bloodstream causing skin diseases, inflammatory conditions, and vascular problems. Raktamoksham removes a small, measured quantity of this impure blood, which stimulates the body to produce fresh, healthy blood. The procedure is performed using sterile disposable equipment under strict medical supervision.

What to Expect
After a thorough medical assessment, the physician identifies the appropriate site and method. The area is cleaned and prepared. A small quantity of blood (typically 30–100 ml) is removed in a controlled manner. The procedure is relatively quick — 30–45 minutes including preparation. Some patients experience immediate relief from symptoms like itching or burning. A specific diet is recommended for 2–3 days following the procedure. This treatment is performed only when medically indicated and after verifying the patient's suitability.

Raktamoksham — controlled therapeutic bloodletting — is the fifth and most specialised of the classical Panchakarma procedures. It is reserved for conditions where vitiated blood (Rakta dushti) is the primary pathology: chronic skin disorders (eczema, psoriasis, urticaria), pigmentation disorders, gout, varicose veins, certain types of headache, and Pitta-dominated inflammatory conditions that have not responded to internal medication. At Vaidya Vrindavanam, Raktamoksham is performed sparingly and only after thorough evaluation. Two methods are used depending on indication: leech application (Jalauka, for localised skin and joint conditions) and venesection (Siravyadha, for systemic Pitta-blood conditions). All procedures follow strict sterile protocols and are physician-led.

Procedure

Patient assessment
Detailed evaluation including blood counts, coagulation profile, and review of medication history (especially anticoagulants). The treating physician determines suitability, selects the method (leech vs venesection), and identifies the application site.

Site preparation
The skin over the target area is cleansed thoroughly. For leech therapy, sterile medical-grade leeches from controlled supply are prepared. For venesection, sterile equipment and trained Marma-aware practitioners ensure safe site selection (typically away from major vessels and nerves).

Bloodletting procedure
Leeches are applied to the affected site and allowed to draw 5–15 ml of vitiated blood naturally over 20–40 minutes; they detach when satisfied and are not reused. For venesection, a small puncture allows controlled drainage of 30–100 ml of blood depending on the indication.

Post-procedure care
The site is cleaned, a medicated paste applied, and a sterile dressing placed. The patient rests for 30–45 minutes; vital signs are monitored. Specific dietary and medication instructions follow for the next 7 days.

- Leech therapy is largely painless — most patients describe only a mild prick at attachment. Venesection is briefly uncomfortable. Skin condition improvement is often visible within 7–10 days; varicose vein heaviness reduces over 3–4 sessions; chronic gout typically improves over 6–8 weeks of combined Raktamoksham and internal medication.
- Contraindications
- Anaemia, bleeding disorders, pregnancy, immunosuppression, current anticoagulant therapy, very young or very frail patients, and any signs of active infection at the site. We screen carefully — Raktamoksham is offered only when clearly indicated, never as a routine wellness treatment.
